Rapid Response Service
Responding in a timely manner to requests to conduct assessments of need and suitability for people that have already been assessed as requiring residential or nursing care. Our care homes stand ready to work closely with hospital discharge and social work teams when a residential placement has been agreed as the right care solution.
Discharge to Assess (D2A) Services
We stand ready to rapidly design and agree D2A provision with the hospital, CCG or local authority to assist patient flow and to allow more time thorough assessments of need and eligibility to take place. This can be arranged on an ad hoc framework basis utilising existing CHC contracts or through a more formal and bespoke contract arrangement. HC-One has particular expertise in this area as we already run these services around the UK.
