With care homes across England, Scotland and Wales, our national scale means we can mobilise rapidly and work in partnership with provider groups, such as Care England, Scottish Care and  Care Forum Wales, and central and devolved Governments, to help achieve fair settlement for providers and commissioners. We also have a dedicated procurement function to ensure we secure the best value contracts from our suppliers. However, our national scale is complimented by a personalised local approach. Our leadership and regional teams benefit from a wealth of sector expertise which enables us to work collaboratively with local authority and NHS partners to understand their individual needs to develop a tailored service.

Our Commissioned Services Team is a collective of highly experienced professionals, with backgrounds in NHS and Local Authority commissioning, voluntary sector and frontline operations. Our care sector expertise mean we can work closely with health and social care commissioners to understand their local priorities, enabling us to develop and enhance our care home services to meet their individual needs.

A warm, welcoming home this winter

This winter, we are here to help ease pressure on frontline health and social care services by providing high quality care to those who need it most. We are proud to stand with our NHS and local authority partners by offering a full care pathway across our network of homes, to accommodate our Residents’ individual needs.

We will continue to deliver Discharge to Assess (D2A) services (pathways 2 and 3) in an enabling and supportive manner this winter, and beyond. In addition, we provide short-term stays for pathway 1 patients, as well as ongoing personalised care plans for those who require longer term solutions, including planned rehabilitation and reablement. 

HC-One prides itself on having extensive experience in this particular area, having set up and delivered both block contracted and spot placement services over the last five years.
